How to Apply for North Carolina Licensure

The Six Step Plan

Step 1

Using your ONYEN and password, log on to the Program Completer Information: Keeping in Touch database and update your profile. (Forgotten your ONYEN or password?)

Step 2

Complete NC Licensure Application forms:

Form A: Application for a North Carolina License (for add-on and licensure only)
Form U: Licensure Update (for lateral entry – check “Clear a Provisional Area”)
Form V: Verification by Institution (top portion only)
Your license will be mailed to the address you use on these forms.

Step 3

Transcripts: (grade reports are NOT acceptable)
You must submit an official UNC-CH transcript which may be obtained from the Office of the Registrar.

You may submit your order several weeks in advance and have them sent directly to the School of Education Licensure Officer. Mark the order form “Hold until current semester grades are posted to transcript.”

If your license was acquired with coursework from multiple universities, you must also submit official transcripts from all other colleges/universities. You may submit an original or a clear photocopy. It is suggested that you submit original copies whenever possible.

Step 4

Testing information:

All students must submit photocopies of passing Praxis II or School Leadership Licensure Assessment score reports.

Step 5

Processing Fee: Check for $55.00* made payable to “NC Department of  Public Instruction” or a Credit Card authorization form (PDF). NO CASH. 

Lateral entry teachers who currently have a provisional teaching license DO NOT have any additional processing fee.

DPI will not process the application packet and will charge an additional fee if your check/credit card does not clear.  PLEASE NOTE: Your check will not be submitted to DPI until the grade posted transcript is received.

Step 6

Evidence of successful teaching experience:

Lateral entry teachers must submit a satisfactory summative evaluation (TPAI) of your most recent year’s teaching signed by your school administrator.

Licensure-only and Add-on students will fill out a Form S: Student Teaching/Interning Performance: Evaluation by North Carolina Local Education Agency which should be submitted to and signed by their university supervisor, cooperating teacher, and LEA administrator. 

Submit

Submit all documents except the Carolina transcript about two weeks before the end of your final semester of study and make arrangements for the Office of the Registrar to send a transcript directly to the Licensure Officer.

You may turn in your application at any time after completing your program, but we can not initiate the process to officially recommend you for licensure until all documentation is received.
