How to Apply for North Carolina Licensure

It is your responsibility to apply for licensure. The process of applying for licensure is separate from applying for graduation - you must do both.

The Five Step Plan

Step 1 - Application

Complete NC Licensure Application forms:

Form A: Application for a North Carolina License
Form V: Verification by Institution (top portion only)
Your license will be mailed to the address you use on these forms.

Step 2 - Transcripts

Grade reports are NOT acceptable. Official UNC Chapel Hill transcript must be DEGREE DATED. Transcripts may be obtained from the Office of the Registrar. You may submit your order several weeks in advance and have them sent directly to the School of Education Licensure Officer. Mark the order form “Hold until degree is posted to transcript.”

If your degree was earned from multiple universities, you must also submit official transcripts from all other colleges/universities. If your initial teaching license is at the master’s level, you must include degree dated transcripts of your bachelor’s degree. You may submit an original or a clear photocopy. It is suggested that you submit original copies whenever possible.

The Master of Education for Experienced Teachers students need only submit their M.Ed. transcript from UNC Chapel Hill.

Step 3 - Test Reports

If required by your program, submit a photocopy of your Praxis II or School Leadership Licensure Assessment score reports.

Step 4 - Processing Fee

Submit check for $55.00* made payable to “NC Department of  Public Instruction” or a Credit Card authorization form (PDF).  NO CASH.

DPI will not process the application packet and will charge an additional fee if your check/credit card does not clear.

PLEASE NOTE: Your check will not be submitted to DPI until the degree posted transcript is received. Degrees are generally posted by the registrar at the end of May, August and December. 

Step 5 - Submit

Submit all documents except the UNC Chapel Hill transcript about two weeks before the end of your final semester of study and make arrangements for the Office of the Registrar to send a transcript directly to the Licensure Officer.

You may turn in your application at any time after completing your program, but we can not initiate the process to officially recommend you for licensure until all documentation is received.
