How to Apply for North Carolina Licensure

The Six Step Plan

Step 1 - Statement

Your program coordinator should sign a statement that all requirements for licensure have been met. Submit the statement with your licensure application documents.

Step 2 - Application

Complete NC Licensure Application forms:

Form A: Application for a North Carolina License
Form U: License Update (if clearing a provisional license)
Form V: Verification by Institution (top portion only)

Step 3 - Transcripts

Grade reports are NOT acceptable. Official UNC-Chapel Hill transcript must be DEGREE DATED. Transcripts may be obtained from the Office of the Registrar.

If your license was acquired with coursework from multiple universities, you must also submit official transcripts from all other colleges/universities. You may submit an original or a clear photocopy. It is suggested that you submit original copies whenever possible.

Step 4 - Test Reports

All students must submit photocopies of passing Praxis II score reports when required for licensure.

Step 5 - Processing Fee

Submit check for $55.00* made payable to “NC Department of  Public Instruction” or payment by credit card - Credit Card authorization form (PDF). NO CASH.

Lateral entry employees working on a provisional teaching license DO NOT have any additional processing fee.

DPI will not process the application packet and will charge an additional fee if your check/credit card does not clear.

Step 6 - Submit

Submit all documents to the Licensure Officer. We can not initiate the process to officially recommend you for licensure until all documentation is received.
